Indications/Uses
Type 2 NIDDM in adults when dietary measures, physical exercise & wt loss alone are insufficient to control blood glucose.
Dosage/Direction for Use
Initially 30 mg daily, may be increased to 60, 90 or 120 mg daily in successive steps w/ at least 1 mth interval. Max: 120 mg daily. Switching from gliclazide 80 mg 1 tab gliclazide 80 mg is comparable to 1 tab gliclazide MR 60 mg. Switching from another oral antidiabetic agent Initially 30 mg daily.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to gliclazide, other sulfonylureas or sulfonamides. Type 1 diabetes; diabetic pre-coma & coma, diabetic ketoacidosis. Concomitant use w/ miconazole. Severe renal or hepatic insufficiency. Lactation.
Special Precautions
Hypoglycemia; malnutrition, irregular mealtimes, skipping meals, fasting or dietary changes, imbalance between physical exercise & carbohydrate intake, overdosage; certain endocrine disorders eg, thyroid disorders, hypopituitarism, adrenal insufficiency; dysglycemia. Regularly monitor blood glucose levels & glycated Hb levels. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Concomitant use w/ St. John's wort (Hypericum perforatum). Renal & severe hepatic insufficiency. Pregnancy. Elderly.
Adverse Reactions
Hypoglycemia; GI disturbances eg, abdominal pain, nausea, vomiting, dyspepsia, diarrhoea, constipation.
Drug Interactions
Increased risk of hypoglycemia w/ miconazole systemic/oromucosal gel, systemic phenylbutazone, alcohol. Potentiated blood glucose-lowering effect w/ other antidiabetic agents (eg, insulins, acarbose, metformin, thiazolidinediones, dipeptidyl peptidase-4 inhibitors, GLP-1 receptor agonists), β-blockers, fluconazole, ACE inhibitors (captopril, enalapril), H2-antagonists, MAOIs, sulfonamides, clarithromycin, NSAIDs. Increased blood glucose levels w/ danazol, chlorpromazine, glucocorticoids, ritodrine, salbutamol, terbutaline IV, St. John's wort prep. Dysglycemia w/ fluoroquinolones, warfarin.
